The form below can only be submitted by those families who are signed up and using on-line giving to pay their annual giving ($500 alms to the Offertory fund).

This form can be filled out online by school parents who have enrolled in Faith Direct online giving.

If you are a registered Fatima parishioner family who is enrolled in Faith Direct Online Giving you may use this form to submit your annual Registered status form every year between Jan 1- October 31st. This form must be submitted by October 31st every year.

It is a requirement that every family complete 10 hours of volunteer time each year. If you are turning in your form prior to completion of your hours (by Oct 31st) but have hours scheduled, you may still submit your form. These hours are reported based on an honor system. If you are in need of volunteer hour opportunities, please visit the Getting Involved page on our website to sign up for a ministry. Volunteering at the parish or school is acceptable.
